documentation: fix almost duplicate filenames (IO/io-mapping.txt)
Having both IO-mapping.txt and io-mapping.txt in Documentation/ was confusing and/or bothersome to some people, so rename IO-mapping.txt to bus-virt-phys-mapping.txt.
