f32e54b1ff Add exop patch from Pierangelo Masarati and myself. 
This enables the folllowing extended operations:
- passwd
- whoami
- refresh
And the following controls:
- manageDSAit
- paged_results
- ppolicy
- noop
- manageDIT
- permissive_modify
